A. PETUNJUK UMUM
1. Formulir Biodata dan Pendaftaran Peserta Ujian Sertifikasi ini akan dibaca komputer secara opscan. Oleh karena itu, pendaftar harus membaca Buku Panduan ini dengan seksama, sehingga tidak akan membuat kesalahan.
2. Formulir Biodata dan Pendaftaran Peserta Ujian Sertifikasi hanya diisi dengan pensil 2B saja (gunakan pensil 2B yang asli dan hati-hati dengan pensil tiruan). Formulir tidak boleh diisi dengan bollpoint, pen, atau pensil HB.
3. Formulir Biodata dan Pendaftaran Peserta Ujian Sertifikasi tidak boleh kotor, sobek, terlipat, atau basah.
4. Jika terjadi salah pengisian, hapuslah bagi-an/bulatan yang salah dengan penghapus yang baik dan bersih.
5. Jangan sekali-kali menghitamkan bulatan atau menulis sesuatu pada bagian formulir yang tidak diminta untuk diisi.
6. Pada waktu menghitamkan satu bulatan, harus sehitam mungkin tetapi jangan sampai merusak formulir, kemudian perhatikan :
a. Cara mengarsir bulatan harus dihitamkan sampai penuh dan jangan sampai keluar dari garis bulatan.
b. Jangan sampai menghitamkan lebih dari satu bulatan dalam satu kolom, karena isian tersebut otomatis dianggap salah.
c. Periksa ulang apakah antara huruf/angka yang Anda tulis didalam kotak dengan bulatan yang Anda arsir sudah sesuai; atau jangan sampai lupa, Anda sudah mengisi huruf/angka di dalam kotak tetapi tidak mengarsir bulatannya.
7. Semua keterangan yang bertentangan dengan isi pengumuman pendaftaran yang diberikan oleh siapapun adalah tidak benar.

B. PETUNJUK PENGISIAN
Kolom yang harus Anda isi adalah kolom K-01 sampai K-25. Pengisian masing-masing kolom harus mengikuti petunjuk sebagai berikut :

K-01 NOMOR KUOTA
Isilah nomor kuota Formulir Biodata dan Pendaftaran Peserta Ujian Sertifikasi pada tempat yang tersedia sesuai dengan nomor kuota Anda.
Contoh:
Nomor kuota Anda adalah 07091540200101, tulislah secara lengkap 07091540200101.

K-02 NIK/NIP
Isilah NIK/NIP Anda sesuai dengan SK Pengangkatan Anda, pada kolom yang tersedia (12 huruf). NIK nomor identitas guru non-PNS, sedang NIP untuk guru PNS.
Contoh:
NIP Anda adalah 132098120, tulislah secara lengkap dimulai dari sebelah kiri.

K-03 NAMA PESERTA
Isilah nama dan gelar Anda pada tempat yang tersedia. Gelar ditulis di belakang. Nama dan gelar harus sedemikian rupa sehingga dapat tercakup dalam tempat yang tersedia (23 huruf). Susunan dan ejaan harus sesuai dengan nama yang tertulis didalam SK Anda.
Contoh :
Drs. MOHAMMAD ALDI FIRMANSYAH M.Si, supaya tercakup dalam tempat yang tersedia dapat disingkat menjadi :
MOHAMMAD ALDI F DRS MSI atau
MOHAMMAD A F DRS MSI

K-04 TANGGAL LAHIR
Isilah tanggal lahir Anda pada tempat yang tersedia, dengan mengisi tanggal, bulan dan tahun masing-masing 2 angka.
Contoh :
Jika Anda lahir pada tanggal 28 Agustus 1986, maka ditulis 280886

K-05 KABUPATEN TEMPAT LAHIR
Isilah pada tempat yang tersedia, kode kabupaten/kota tempat Anda lahir, sesuai Daftar Kode Kabupaten/Kota di dalam Buku Petunjuk ini.
Contoh:
Tempat lahir anda di Kabupaten Tapanuli Tengah, Propinsi Sumatera Utara, maka kode kabupaten diisi : 09

K-06 PROPINSI TEMPAT LAHIR
Isilah pada tempat yang tersedia, kode propinsi tempat Anda lahir, sesuai Daftar Kode Propinsi di dalam Buku Petunjuk ini.
Contoh:
Tempat lahir anda di Kabupaten Tapanuli Tengah, Propinsi Sumatera Utara, maka kode propinsi diisi : 07

K-07 JENIS KELAMIN
Hitamkan satu bulatan yang sesuai dengan jenis kelamin Anda.

K-08 PENDIDIKAN
Hitamkan satu bulatan yang sesuai dengan jenjang pendidikan Anda.

K-09 TAHUN LULUS S1/D4
Isilah pada tempat yang tersedia, tahun yang tercantum pada ijazah S1/D4 yang Anda miliki (2 angka).
Contoh
Jika Anda lulus S1/D4 pada tahun 1986, maka ditulis 86 (2 angka terakhir).

K-10 AKTA
Hitamkan satu bulatan yang sesuai dengan kepemilikan Sertifikat Akta Mengajar.
Contoh:
Jika Anda sudah mempunyai Sertifikat Akta Mengajar, maka hitamkan bulatan Ya. Sebaliknya jika belum mempunyai Sertifikat Akta Mengajar, maka hitamkan bulatan Tidak.

K-11 BIDANG STUDI S1/D4
Isilah kode bidang studi waktu Anda menempuh jenjang pendidikan S1/D4, pada tempat yang tersedia (3 angka), sesuai Daftar Kode Bidang Studi di dalam Buku Petunjuk ini, bila tidak ada dalam daftar, tulislah dengan angka 999.
Contoh:
Jenjang S1 Anda adalah Bahasa Inggris, ditulis 157.

K-12 BIDANG STUDI MENGAJAR
Isilah kode bidang studi yang Anda asuh disekolah tempat Anda mengajar sekarang, pada tempat yang tersedia (3 angka), sesuai Daftar Kode Bidang Studi di dalam Buku Petunjuk ini, bila tidak ada dalam daftar, tulislah dengan angka 999.
Contoh:
Anda mengajar di TK umum, cukup ditulis 020.

K-13 NAMA SEKOLAH TEMPAT MENGAJAR
Isilah nama sekolah tempat Anda mengajar pada tempat yang tersedia. Nama sekolah harus sedemikian rupa sehingga dapat tercakup dalam tempat yang tersedia (23 huruf). Jika nama sekolah melebihi tempat yang tersedia, maka singkatlah dengan singkatan yang mudah dimengerti atau sudah umum dipakai.
Contoh :
SMU SRI LANGKAT, TANJUNG PURA, supaya tercakup dalam tempat yang tersedia cukup ditulis: SMU SRI LANGKAT TG PURA

K-14 ALAMAT SEKOLAH TEMPAT MENGAJAR
Isilah nama alamat sekolah tempat Anda mengajar pada tempat yang tersedia (23 huruf) dengan singkat dan jelas, yang dapat dijangkau oleh pos. Jika alamat sekolah melebihi tempat yang tersedia, maka singkatlah dengan singkatan yang mudah dimengerti atau sudah umum dipakai.
Contoh :
JL MAYJEN PANJAITAN X/86 MALANG Ds. Karangduren, Kec. Pakisaji, Kab Malang, supaya tercakup dalam tempat yang tersedia cukup ditulis : KR DUREN PAKISAJI MALANG atau JL PANJAITAN X/86 MALANG.

K-15 NOMOR KUOTA
Isilah nomor kuota Formulir Biodata dan Pendaftaran Peserta Ujian Sertifikasi pada tempat yang tersedia sesuai dengan nomor kuota Anda

K-16 PENGALAMAN MENGAJAR
Isilah bulan dan tahun mulai Anda mengajar, sesuai dengan SK Pengangkatan, pada tempat yang tersedia, dengan mengisi bulan dan tahun masing-masing 2 angka.
Contoh:
Jika Anda mulai mengajar sesuai dengan SK Pengangkatan pada bulan april tahun 1990, maka ditulis 0490.

K-17 BEBAN MENGAJAR
Isilah dengan jumlah beban jam mengajar Anda dalam satu minggu, pada tempat yang tersedia (2 angka).
Contoh:
Anda mengajar dalam satu minggu sebanyak 42 jam, ditulis 42.

K-18 ALAMAT RUMAH
Isilah nama alamat rumah (tetap) Anda pada tempat yang tersedia (26 huruf) dengan singkat dan jelas, yang dapat dijangkau oleh pos. Jika alamat rumah melebihi tempat yang tersedia, maka singkatlah dengan singkatan yang mudah dimengerti atau sudah umum dipakai.
Contoh :
JL MAYJEN PANJAITAN X/86 MALANG Ds. Karangduren, Kec. Pakisaji, Kab Malang, cukup ditulis : KR DUREN PAKISAJI MALANG atau JL PANJAITAN X/86 MALANG.

K-19 NAMA PERGURUAN TINGGI
Isilah nama perguruan tinggi dimana Anda menempuh jenjang pendidikan S1/D4, pada tempat yang tersedia (20 huruf) dengan singkat dan jelas. Jika nama perguruan tinggi melebihi tempat yang tersedia, maka singkatlah dengan singkatan yang mudah dimengerti atau sudah umum dipakai.

K-20 NOMOR KUOTA
Isilah nomor kuota Formulir Biodata dan Pendaftaran Peserta Ujian Sertifikasi pada tempat yang tersedia sesuai dengan nomor kuota Anda

K-21 JENIS TENAGA KEPENDIDIKAN
Hitamkan satu bulatan yang sesuai dengan jenis tenaga kependidikan Anda.

K-22 JABATAN STRUKTURAL
Hitamkan satu bulatan yang sesuai dengan jabatan struktural Anda.

K-23 PRESTASI TERTINGGI
Hitamkan satu bulatan yang sesuai dengan prestasi Anda.

K-24 TAHUN MEMPEROLEH PRESTASI
Isilah tahun Anda memperoleh prestasi, pada tempat yang tersedia (2 angka).

K-25 PERNYATAAN
Salinlah pada tempat yang tersedia pernyataan :
Dengan ini saya menyatakan bahwa data yang diisikan dalam formulir ini adalah benar.
Dengan tulisan biasa, bukan huruf cetak. Kemudian tulislah nama terang dan tanda tangan anda.

I deem it a great honor and privilege to be called upon to deliver the keynote address for this workshop, which is to deliberate on how the cultures of public-sector organizations in Ghana can affect their development, survival and overall worth to the communities these organizations are supposed to serve.
Mr. Chairman, over the years researchers have increasingly investigated organizational culture and the possible links to organizational performance and effectiveness. According to some, organizational culture, more than any other factor, dictates an organization’s ability to survive and succeed.
If this organizational culture would be so important for the survival and success of public-sector organizations, we would like to understand what it is all about. With out getting too theoretical, let us say that organizational culture refers severally but commonly to the “shard pattern of beliefs, assumptions and expectations” of the organization’s members. In the words of Hofstede (1995).” Culture is collective programming of the mind which distinguishes one group or category of people from another.” Within on organization this is defined by group norms, espoused values, rulers of the game, habits of thinking, mental models, attitude to time, etc. In essence, an organization’s culture is the repository of what its members agree about. Organizational culture affects all aspects of organizational life from the ways in which people interact with each other, perform their work and dress, to types of decisions made in an organization, its policies, procedures and strategy considerations; how top management deals with various situations ; how people actually spend their time at work; and the organization’s physical setting, etc.
In our local context, organizational culture may seen in :
- The attitude toward leadership and style. For example. We tend to put our leaders on pedestals and accept high hierarchical distance between the governors and subordinates, In companies in America, for example, the managing director and a messenger may eat at the same “takeaway” and ride in the same bus.
- Attitude to time management : Ghana meantime or Greenwich meantime.
- Work planning and performance monitoring versus “aban adwuma” (government work ) mentality.
- Propensity to consume, not to save invest.
- “Connection” versus merit system, etc.

The point that I am trying to make is that organizational values obtained from the culture are important because a firm or institution’s underlying values and beliefs define the organization’s philosophy for achieving success. Thy reflect the view of the way “ things should be” in an organization that is shared by organizational members. That philosophy will provide a sense of common direction for its members and guidelines as to acceptable behaviors in their daily operations.
Positive organizational culture have been linked to increased staff alignment, resulting in enhanced organizational effectiveness, heightened consensus regarding strategic direction.
Mr. Chairman, we thing that leadership and organizational culture are tightly intertwined. Leaders must have a deep understanding of the identity and impact of the organizational culture in order to communicate and implement new visions and inspire follower commitment to the vision ( Schein, 1990) Transformational leaders help shape and maintain the desired culture of an organization ( Schein, op cit), which may link to organizational effectiveness. Some researchers have suggested that transformational leadership and organizational culture contain the key to understanding organizational effectiveness ( base and Avolio, 1992)
High transformational leaders possess strong organizational cultures and carry out culture-building activities, especially the customer-orientation function, to a greater extent then other leaders do. Yuk (1994) defined transformational leadership as the process of influencing major changes in the attitudes and assumptions of organizational members and building commitment for the organization’s mission, objectives and strategies (p. 271).
The current thinking in the area of leadership is devoted to the leader’s role in maintaining the organizational culture or in changing it to implement a change of direction dictated by a new vision ( Bryman, 1992) . The researcher suggested that leader could alter or impact the organizational culture.
Executive leaders should put their energies into developing a strong organizational culture that supports activities such as managing change, achieving goals, coordinating teamwork , and customer orientation in organization. These activities contribute to organizational effectiveness. Successful organizations, over time, are likely to possess a strong, well-defined culture. Organizational culture must support activities linked to the mission of the organization.
Mr. Chairman, I would now like to turn my attention to public-sector organizations and organizational culture with particular reference to Ghana. Traditionally we have the civil service, which one may label the secretariat of government and other state-owned institutions. While they share some similarities, they differ in many ways and as such will be advisable to speak of the civil service and public sector organizations with the latter referring to entities other than the civil service.
The service inherited a unique culture from colonial times. The system has no doubt been disturbed, especially by the curse of military interventions and so called revolutions. In the main its culture remains the same, sadly to say with some worsening trends after the old crop of professional, committed and industrious public spirited civil servants of the A.I . Adu and Debrah “types” moved on. The Culture of the service in recent times is not something to write home about . It is still a rule base system; managerial leadership has been weak, and over the years the civil service got political. Especially with the introduction of political units from 1882 and politically appointed chief directors.
The situation is compounded by poor conditions of service, logical limitations and a less than ideal elected official civil servant relationship. The whole culture of the civil service requires urgent attention.
Lately one hears of s “ chicken and egg” argument. The leadership of the service argues that all will be ”kosher” if civil servants are paid well. On the other hand, some of the authorities complain of poor work ethics and attitudes that undermine government policy and programs. The situation is compounded by a half-baked decentralization that has resulted in the creation of a parallel system . But this debate cannot go on forever. Urgent action is needed.
It is my considered view that improving the culture of the civil service will require more than a uni dimensional action of salary increase. Hard as it is to implement, it requires a combination of effective managerial leadership, competency based training, a performance-linked incentive system, and improved conditions of service. To ask a government which already spends the greater share if its revenues on public-sector wages and salaries to pay more is quite simplistic. This is particularly so in a situation whereby devoting 100 percent of current government revenues will still pay salaries which most of us will agree will be inadequate. Thus there is the need to link together revenue collection, economic growth and improvement in public-service wages in the long run.
As to the wider public service, I have a stronger message. Since they tend to be well-defined entries a university, a secondary school, a factory, a service unit- the scope for a positive revolutionary culture change is greater. I say revolutionary because we are dealing with ingrained traditional attitudes militating against affective time management, productivity “ aban adwuma”(government work – which is minimum effort for maximum gain), and a high propensity to cheat and steal ( which we have given a wrong label as corruption). Reversing organizational culture in such a milieu is both doable and challenging.
I believe the key to that is effective transformational leadership, which provides vision, goals and discipline as well as models core moral and ethical values. I will require managers “ who walk their talk,” and this will gain the confidence of their staff to manage change.
Unfortunately , the way leadership of public institutions is appointed makes that difficult. In the first place, the tendency is to appoint boards based on party affiliation, old school boyism and girlism, etc. I must add that the situation has improved markedly in recent times. What has not changed is the simultaneous appointment of boards and CEOs with the result that CEOs don’t feel accountable to their boards but appointing authorities. The situation is the same whether one is talking about government appointments or those appointed by constitutional bodies such as the National Media Commission. Unless organizational culture improves, we cannot expect the landscape of corporate governance to improve.
Another factor that constrained improvement in organizational cultures in the public sector has to do with our socialist past, of seeing these entities as employment bureaus, and the fear also of declaring redundancies and insistence on doing a day’s work for a month’s pay, etc. It is clear that the public sector is not a major employer of Ghanaians; it employs only about 800,000 out of a workforce of about 10 million. Yet the efficiency of the civil service and public sector will determine the growth of the economy and employment generation of the private sector. Thus, it is incumbent on us to do whatever it takes to improve the culture of the public service, generate growth and improve overall levels of employment in the economy.
I believe that we can grow the economy of Ghana at double digit rates, though it will require more than World Bank IMF straight jacketed policies. That would require a streamlined, capable, motivated, disciplined and accountable civil service and public sector. And the key to that is better organizational leadership and culture.
Chairman, let me conclude by making reference to efforts at public sector reform in Ghana during the past four years largely under the National Institutional Renewal Program (NIRP) . ( Other parallel system included Civil Service Performance Improvement Program. Reform of the judiciary. Etc.) In the main it was largely a consultant driven process, some times with as much as half a million dollars spent on consultants just to diagnose what is wrong and needed to be done but with nothing spent on fixing any problem.
The leadership of the organization under reform was either bypassed or treated as suppliers of information to mushroomed consultants. Overnight, consultants who have not had any experience of running any institution in their lives pocketed thousands of dollars, part of which could have ended as kickbacks to hose awarding the contracts, given the rates at which contracts were awarded and the low quality control in some cases. In the end, only one institution got itself off subvention and another GRATIS (Ghana Regional Appropriate Technology Industrial Service) is today heading toward self financing.
The lessons for future are clear . There will be no transformation for public sector organizations without changes in organizational culture. In that regard the managerial leadership of the public sector is “ cause ; everything else is effect.” That government is retooling public reform. The emphasis has to be placed on organizational culture change.

Organizational culture is a commonly held –in-the-mind framework of organizational members. This framework contains basic assumption and values. These basic assumption and values are taught to new members as that organizational culture is developed over time as people in the organization learn to deal successfully common background. So culture arises out of what has been successful for the organization.

More about Organizational Culture

Culture starts with leadership, is reinforced with the accumulated learning of the organizational members, and is a powerful (albeit often implicit) set of forces that determine human behavior.

An organization’s culture goes than the word used in its mission statement. Culture is the web of tacit understandings, boundaries, common language, and shared expectation maintained over tome by the members.

Way of looking at organization culture originally come out anthropology. He are some aspects of culture :

Historical : Culture is social heritage, or tradition is passed on to future generation
Behavioral : Culture is shared, learned human behavior. A way of life
Normative : Culture is ideals, values, or rules for living
Functional : Culture is the way people solve problems of adapting to the environment and living together
Mental : Culture is a complex of ideas, or learned habits, for social control
Structural : Culture consists of patterned and interrelated ideas, symbols, or behaviors
Symbolic : Culture is based on arbitrarily assigned meanings that are shared by an organization

Model of Organizational Culture

To really be able to characterize and “speak” and organization’s culture a person would need to be able to step back objectively and do some critical observation and interviews. Various researchers have developed models to characterize culture, both qualitatively and quantitatively.

Hofstede probably started it all

Geert Hostede is an engineer turned social scientist. After 10 year working as an engineer and manager in Dutch Industry, he returned to the university to study social psychology. His groundbreaking 1980 book, Culture’s Consequences (reprinted afresh in 2001) grew out of his research within IBM from 1973 to 1978. from what was at that time the word’s largest survey data base. Hofstede and his colleagues teased out differences in the mental program among over 115,000 IBMers across 50 nations, and laid the groundwork for other scholars to adapt his work and use it to study organizations. Hofstede’s work identified five major dimensions upon country cultures differed:

• Power distance – how hierarchies and unequal power distribution is viewed
• Uncertainty avoidance – the extent to which people are comfortable or uncomfortable with uncertainty
• Individualism – this is the anchor at one end two people, where the other anchor would be collectivism. This is the extent to which individual are supposed to be self-reliant and look after themselves. Versus being more integrated into a group.
• Masculinity or Femininity – the dimension that has probably caused the most uproar. This dimension reflects hardness vs softness; toughness vs tenderness in a culture.
• Long tern or short term orientation – this has to do with the culture’s members having a stance on delayed, or immediate

Hofstede noted in his writing that it is important to recognize that national culture and organizational culture are different in nature. His research indicates that national culture mostly stems from consistency in values; while organizational culture stems mostly from consistency in practices.

Piggybecking from Haofstede

Management researcher were quick to adapt Hofstede’s work and begin to investigate cultures inside organizations. O’Reilly, Chatman, and Caldwell (1991) in some comparative work published that seven dimension could be used to compare across organizations.

Some Dimensions of Organizational Culture

• Innovation and risk talking – willing to experiment, take risk, encourage innovation.
• Attention to detail – paying attention to being precise vs saying its “good enough for chopped salad”
• Outcome orientation – orientated to result vs oriented to process
• People orientation – degree of value and respect for people. Are people considered unique talents, or is an engineer an engineer
• Individual vs Team orientation – are individuals most highly notes. Or are collective efforts.
• Aggressiveness – taking action, dealing with conflict
• Stability – openness to change

A. Composite Two-by-Two
Some researchers present a framework of culture characterized by two dimensions
• Internal focus (attending primarily to what is going on inside the organization) vs External focus (attending primarily to what is going on outside the organization
• Stability and control [interest in keeping things the same) vs Flexibility and discretion (interest in making changes)

Internally focused with Flexibility and Discretion
This type of organization has a sense of cohesion, with goals that strongly shared. Inside, the organization may feel more ‘family like” than ‘business like”. Indeed, Cameron and Quinn call this a Clan Culture. Denison tags this type of organization as having Involvement Externally focused with Flexibility and Discretion
The emphasis on being open change and oriented to the outside word characterizes organizations in which innovation can thrive, indeed sometimes the innovativeness can run amuck. Cameron and Quinn call these Adhocracy Cultures. Denison characterized them as high Adaptability culture.
Internally focused with Stability and Control
This type organization often relies on formal structures, policies and procedures to keep thing rinning. An internal focus is on Consistency says Denison. Cameron and Quinn named this type Hierarchy Culture Externally focused with Stability and Control
These types of organization are concerned about productivity, consistency, results, the button line. These organization are very clear about their customers, and hence can be termed Marked Culture. Denison says these organization have a sense of external Mission, combined with control, that can be very successful.

Looking Organization Culture – Use both Qualitative and Quantitative

In academia the scholars interested in organizational culture have kept a small-fire war going form years discussing the pros and cons of qualitative or quantitative ways of looking at culture. The qualitative camp point out that the richness of perception and experience inside an organization are vital to deep understanding, and they sniff that culture cannot be constrained to a two matrix or a list of dimensions. On the other camp, quantitative researcher argue that managers need to have some hard data, and that the drawback of getting slow, expensive, possibly unreliable (unique to the interpretation of the researcher) qualitative information make the usefulness iffy at best.

The truth, of course, lies in the middle. Managers will be best served by both. Case studies, based on observation and insider interviews, have a sense of reality and immediacy that captures the attention and emotion. Observation of the components of culture, with discussion and analyses, offer ways to do qualitative tracking over time. Having a method for obtaining quantitative data has the advantage of allowing managers to put together more “hard data” analyses to look at culture as a component of management and to track the standardized capture components of culture longitudinally. Look at organization using data gathered in a variety of methods, or triangulation. Combines quantitative and qualitative data that allows managers to capitalize on the advantages of quantitative methods as well as capturing a rich no easily quantified picture of the organization

Bringing in outside eyes, qualified academics or consultants, can be a helpful way for managers to begin to look at the organizational culture. It is not the only way, however. People can engage in developmental processes to help them selves recognize aspects of own organization culture.

A Qualitative understanding can be developed by looking at organizational practices with a fresh set of eyes. To begin to understand culture, put on an anthropologist’s hat and …..

- How are people dressed?
- Where do they eat lunch ?
- How would you characterize the people in the hall- formal or informal?
Laughing
- What kinds of pictures, sign, jokes are on walls Listen for particular language.

Notice the Feature of Culture
- Ceremonies, Rites, and Rituals
- Stories and Myths
- Herpes
- Language
- Symbols Ask Question. Like :
- Tell me the organizational creation story
- How do new people “learn the ropes “ in the organization
- What gets noticed (and rewarded )?
- Are some people on the “fast track,” and if so. How did they get there?
- What are some taboos thing people should never do?
- If a team accomplishes something great, what happens?
- Describe the organization in there words
- If the organization were an animal what would it be ?

• Ceremonies, Rites and Rituals These are regular events that teach people about the culture, and maintain a sense of seasonal order. Think about quarterly site meetings; any rites and rituals associated with being promoted, annual sales meetings.
• Stories and Myths Organizations develop narratives to explain and teach . Common examples are stories told by managers about successes, failures, high visible actions. Stories usually have a basis in reality; myths can be fictitious, but so indicative of the culture that people tell and retell them anyway.
• Herpes Organizations develop internal heroic figures.
• Language Organizations develop acronyms, metaphors, proverbs, and jargon that have specific meaning inside.
• Symbols Signs, company logos, the way offices look, the type of clothing that is allowed are all symbolic reflectors of a culture.

Let’s touch on Quantitative

The most common method for quantitatively capturing culture information is through the use of survey assessments. A number of consulting firms offer services related to such surveys. Before closing to develop a relationship with a consulting firm, it is important to examine their credentials in terms of academic grounding validity and reliability of the instrument, and experience and knowledge of this type of assessment.

On the other hand, if an organization has knowledgeable internal research group, using the Organizational Culture Assessment Instrument (OCA) developed by Cameron and Quinn (1999) would provide useful and valid information. Another often used assessment is the Organizational Culture Index (OCI) developed by Robert Cooke and Clayton Lafferty. This assessment uses questions about behavioral norms. Falling into 12 factors. Cooke and his associates have developed a circumplex plotting process, which then categorizes the organization as being one of three types : Constructive. Passive/Defensive, and Aggressive /Defensive.

Because of their deep knowledge of the area, and their expertise with research. Daniel Denison’s consulting firm would certainly be a good group to at least talk with (htt://www.denisonculture. com/culture/culture_main.htm)n Another often called upon consulting group is Human Synergistic (http:www.humansyn.com/) which uses the OCI and other assessments developed by Cooke and Lafferty.

Both of these consulting firm have a large database against which they will copare any client’s scores. That may or may not fit the particular question of an organization. A more useful, butt tougher task, would be developing some connections between the outputs of a survey and the need for innovation within the organization

Innovation is the development and introduction of a new idea and transforming that idea into a product, process, object, or service. We would likely all agree that innovation is the life force of an organization. NUPTK

NUPTK atau Nomor Unik Pendidik dan Tenaga Kependidikan adalah nomor yang diberikan kepada setiap individu yang sedang bekerja sebagai pendidik dan tenaga kependidikan formal maupun non formal yang bersifat unik secara nasional.

Pendidik dan tenaga kependidikan yaitu :
Guru, Kepala Sekolah, Laboran, Pustakawan, Penjaga Sekolah, Tutor, Pengawas Sekolah, Instruktur Khusus, dsb bidang kependidikan.

Dalam rangka mendukung program Direktorat Jenderal Peningkatan Mutu Pendidik dan Tenaga Kependidikan (Ditjen PMPTK) dalam memenuhi tuntutan UU SISDIKNAS NO.20 TH 2003 dan UU GURU DAN DOSEN NO.14 TH 2005 yaitu :
1. Pemberian tunjangn khusus bagi PTK
2. Melakukan Sertifikasi Guru
3. Memberikan Penghargaan Akhir masa Bakti
4. Memberikan beasiswa bagi anak guru berprestasi
5. Memberikan Peningkatan Mutu Pendidik
6. Program Pembangunan Rumah Dinas Kepala Sekolah dan Pengawas Sekolah
7. Mendistribusikan dana blockgrant guru bantu, pengawas sekolah, blockgrant operasional pengawas sekolah
8. Pemberian tunjangan fungsional Guru non PNS
9. Pemberian Tunjangan Kelebihan Jam Mengajar
10. Peningkatan kualifikasi Guru dari D-4 ke S1

Maka sejak Desember 2006 dilakukan pendataan NUPTK kepada seluruh PTK (Negeri dan Swasta) di Indonesia melalui LPMP dan Dinas pendidikan Kabupaten/Kota.

Tahapan Pendataan NUPTK adalah :
1. Pengebaran instrumen profil NUPTK (5 lembar untuk masing-masing PTK) oleh dinas pendidikan Kabupaten/Kota. PTK mengisi profil NUPTK dengan lengkap di sekolah induknya.
2. Pengumpulan instrumen profil NUPTK
3. Pengentrian data ke program aplikasi SIMNUPTK
4. Pengiriman data ke LPMP
5. Validasi data di LPMP
6. Proses penomoran Ditjen PMPTK Jakarta.

Demi terealisasinya program tersebut dan juga untuk kepentingan para PTK, maka perlu dilakukan verifikasi data untuk memastikan bahwa PTK sudah terdata dalam data base dan sudah memiliki Nomor Unik Pendidik dan Tenaga Kependidikan (NUPTK). seluruh data harus diserahkan ke Ditjen PMPTK pada Bulan September 2007 untuk diberikan Nomor Unik Pendidik dan Tenaga Kependidikan (NUPTK). Dikarenakan banyaknya Data yang belum masuk kami (LPMP) terima dari Dinas Pendidikan Kabupaten Garut, maka Ditjen PMPTP memberikan waktu sampai dengan akhir Nopember 2007 untuk menyelesaikan data yang tersisa dan divalidasi di bulan Desember 2007.

Bagi PTK yang belum terdata, harus mengisi instrumen Profil NUPTK terlebih dahulu disertai keterangan mengajar dari pimpinan sekolah yang menjadi sekolah inti/induk ke Dinas Pendidikan kabupaten, kemudian dientri ke program SimNUPTK melalui Tim DataBase Dinas Pendidikan Kabupaten. Untuk diketahui bahwa Program Ditjen PMPTK akan segera direalisasikan pada Tahun 2008

NOMOR UNIK PENDIDIK DAN TENAGA KEPENDIDIKAN 2008 dapat diketahui dengan cara mencoba MASUK KE MENU DOWNLOAD UNTUK MELIHAT DAFTAR NUPTK di www.nuptk.info. Tim ICT Direktorat Jenderal Peningkatan Mutu Pendidik dan Tenaga.

Berkenaan dengan program Data Pokok Pendidikan (DAPODIK) khususnya Nomor Unik Pendidikan dan Tenaga Kependidikan (NUPTK). Kami menghimbau kepada seluruh masyarakat Indonesia serta seluruh guru (tenaga pendidik) dan karyawan (tenaga kependidikan) di seluruh Indonesia untuk turut berperan aktif dalam pemantauan data yang ditampilkan secara online di situs nuptk

Adapun untuk masukan dan laporan dari masyarakat dapat ditujukan langsung ke Lembaga Penjamin Mutu Pendidikan (LPMP) di wilayah kerja masing-masing. Peran aktif masyarakat akan sangat membantu proses validasi dan akurasi data-data yang dimaksud untuk masa depan online bagi kemajuan dunia pendidikan di Indonesia.

Untuk mendapatkan NUPTK setiap PTK tidak dikenakan beban biaya (GRATIS) hanya saja wajib mengisi Format Instrumen NUPTK dengan lengkap dan benar serta di entri kedalam data base Sim-NUPTK, keberhasilan PTK mendapat NUPTK tergantung pada cara pengisian oleh PTK dan entri data oleh  sekolahnya masing-masing.

PENTINGNYA NUPTK BAGI PENDIDIK DAN TENAGA KEPENDIDIKAN

Undang-undang Nomor 20 tahun 2003 tentang Sistem Pendidikan Nasional, Undang-Undang Nomor 14 tahun 2005 tentang Guru dan Dosen, dan Peraturan Pemerintah Nomor 19 tahun 2005 tentang Standar Nasional Pendidikan mengamanatkan bahwa guru adalah pendidik profesional, seorang guru atau pendidik profesional harus memiliki kualifikasi akademik minimal sarjana (S1) atau Diploma empat (D4), menguasai kompetensi, memiliki sertifikat pendidik, sehat jasmani dan rohani, serta memiliki kemampuan untuk mewujudkan tujuan pendidikan nasional.

Untuk melaksanakan amanat Undang-Undang, maka Pemerintah melalui Departemen Pendidikan Nasional memprogramkan diantaranya; Sertifikasi bagi Guru dalam Jabatan, Subsidi Peningkatan Kualifikasi Guru ke S1/D4, Peningkatan Kompetensi Guru, dan sebagainya.
Tahun 2008 secara nasional Depdiknas menetapkan kuota sertifikasi guru dalam jabatan sebanyak 200.000 guru dan provinsi Jambi sebanyak 3.322 orang, dasar penetapan kuota per-provinsi adalah jumlah guru di provinsi masing-masing berdasarkan SIMPTK tahun 2007 sehingga cara seperti ini sudah tentu proporsional karena secara otomatis provinsi yang jumlah gurunya banyak maka kuotanya juga banyak begitu sebaliknya.

Dalam menentukan calon peserta sertifikasi guru tersebut diatur berdasarkan petunjuk teknis sertifikasi guru oleh Direktorat Jenderal Peningkatan Mutu Pendidik dan Tenaga Kependidikan (Ditjen PMPTK) yaitu melalui data yang ada pada SIM NUPTK yang dikelola oleh Ditjen PMPTK, LPMP, dan Dinas Pendidikan Kabupaten/Kota.

Mengingat beberapa program terkait dengan guru (sertifikasi, kualifikasi, peningkatan kompetensi, tunjangan kemaslahatan, dsb.) maka SIM NUPTK memiliki arti dan makna strategis dalam mendukung program dan kebijakan Pemerintah khususnya di bidang pendidikan, yang jelas bahwa dalam pelaksanaan 9 program unggulan Ditjen PMTPK berkaitan dengan NUPTK, yaitu :

1. Program Peningkatan Kualifikasi Guru;
2. Program Sertifikasi Profesi Guru;
3. Program pengembangan mutu PTK-PNF;
4. Program Tunjangan Guru (fungsional, profesi, khusus dan maslahat tambahan);
5. Program penghargaan dan perlindungan PTK;
6. Program perencanaan kebutuhan, keseimbangan, penempatan dan pengembangan karir PTK;
7. Program peningkatan profesionalitas guru berkelanjutan;
8. Program pembinaan Tenaga Kependidikan;
9. Program penguatan kinerja PMPTK;

Dasar hukum pelaksanaan pendataan PTK melalui NUPTK, adalah :

1. Undang-Undang Dasar Republik Indonesia Tahun 1945: Pembukaan pada alinea 4, dan pada Bab XII Pendidikan, pasal 31, ayat (1) dan (2)
2. Undang-Undang Republik Indonesia Nomor 20 Tahun 2003 tentang Sistem Pendidikan Nasional, Bab XI Pendidik dan Tenaga Kependidikan, pasal 30 sampai pasal 44
3. Undang-Undang Republik Indonesia Nomor 32 Tahun 2004 Tentang Pemerintah Daerah
4. Undang-Undang Republik Indonesia Nomor 33 Tahun 2004 Tentang Perimbangan Keuangan antara Pemerintah Pusat dengan Pemerintan Daerah
5. Undang-Undang Republik Indonesia Nomor 14 Tahun 2005 Tentang Guru dan Dosen
6. Peraturan Pemerintah Republik Indonesia Nomor 19 tahun 2005 tentang Standar Nasional Pendidikan
7. Peraturan Presiden Republik Indonesia Nomor 7 tahun 2005 tentang Rencana Program Jangka Menengah Kementrian Negara Republik Indonesia
8. Peraturan Menteri Pendidikan Nasional Republik Indonesia Nomor 8 tahun 2005 tentang Kedudukan, Tugas, Fungsi, Susunan Organisasi, dan Tata Kerja Direktorat Jenderal Peningkatan Mutu Pendidk dan Tenaga Kependidikan
9. Peraturan Menteri Pendidikan Nasional Republik Indonesia Nomor 7 tahun 2007 tentang Organisasi dan Tata Kerja Lembaga Penjaminan Mutu Pendidikan.

Ditjen PMPTK juga memberikan Nomor Unik Pendidik dan Tenaga Kependidikan (NUPTK) yang terdiri dari 16 digit numerik dan bersifat unik kepada PTK yang memiliki informasi yang baik dan lengkap, baik dimaksud adalah data rasional misalnya riwayat pendidikan tahun masuk dan tamat sesuai dan benar (logis) dan lengkap dimaksud adalah secara pokok data yang bersifat tetap/tidak berubah terisi lengkap yaitu nama, tanggal lahir, jenis kelamin, sekolah induk, nama ibu kandung, dan riwayat pendidikan (terutama Sekolah Dasar) lengkap dengan tahun masuk dan tamat.

Sistem pemberian nomor ini juga dilengkapi dengan proses pencarian data PTK yang terhitung ganda (double-counting) akibat mengajar di beberapa sekolah atau bekerja di beberapa instansi pendidikan untuk menghasilkan informasi tabulasi jumlah PTK secara riil. Namun dengan hasil report SIM NUPTK dalam bentuk analisis jumlah PTK tidak terjadi perhitungan ganda, hal ini dimisalkan ada seorang guru yang mengajar di dua sekolah (katakanlah SMP 1 dan SMP 2), ketika kedua sekolah tersebut sama-sama menyampaikan laporan secara manual jumlah gurunya maka sudah pasti seorang guru tersebut sama-sama dimasukan dalam laporan masing-masing sekolahnya sehingga jumlahnya akan terduplikasi, nah dengan SIM NUPTK hal ini tidak terjadi karena SIM NUPTK mampu mendeteksi data ganda melalui deteksi sekolah induk, tanggal lahir, jenis kelamin, tanggal kelahiran, nama ibu kandung, dan riwayat pendidikannya.

Sedangkan Guru yang bersangkutan dapat mengisi format NUPTK di kedua sekolah tersebut dengan kiat salah satu sekolah yang menjadi sekolah induk harus di checklist sedangkan sekolah lainnya jangan di checklist jika hal ini dichecklist atau tidak dichecklist kedua sekolah tersebut maka ketika akan dientry ke SIM NUPTK akan terbaca data bahwa data yang bersangkutan sudah pernah ada…. nah tim entry data akan menentukan dimana salah satu sekolah yang menjadi sekolah induknya.

Lantas apa kegunaannya Guru tersebut boleh mengisi dua instrumen di sekolah masing-masing, hal ini bertujuan untuk menghitung beban mengajar guru. Jadi jika guru mengajar di SMP 1 sebanyak 16 jam dan di SMP 2 sebanyak 10 jam maka total Guru tersebut mengajar menjadi 26 jam. Hal ini sama saja jika Guru hanya mau mengisi satu instrumen NUPTK di sekolah induknya saja tapi dengan syarat pada bagian 2 kolom mengajar di sekolah lain harus diisi dengan lengkap.
Dalam hal pencarian data individu melalui SIM NUPTK berguna untuk menentukan calon peserta yang akan mendapatkan program Ditjen PMPTK, sebagai contoh : program sertifikasi bagi Guru. Karena dalam melakukan pencarian data dapat langsung menampilkan data nama Guru, pendidikan, masa kerja, usia, pangkat/golongan (untuk PNS), beban mengajar, tugas tambahan, unit kerja, dan sebagainya secara cepat dan akurat dengan syarat data individu Guru dimaksud terisi dengan lengkap dan benar, sehingga untuk menentukan calon peserta sertifikasi guru dapat dilakukan dengan mudah dan benar. Selanjutnya data tersebut digunakan untuk melakukan urutan sesuai dengan peringkat yang ditentukan dalam penentuan peserta sertifikasi Guru.

Disamping membantu dalam proses penentuan data calon peserta sertifikasi Guru, NUPTK juga dapat dijadikan sebagai pencarian data bagi pelaksanaan program-program lainnya, misalnya : kualifikasi, peningkatan kompetensi tunjangan kemaslahatan, dan sebagainya. Caranya adalah :

• Untuk melaksanakan program peningkatan kualifikasi Guru, sebelumnya dapat ditelusuri terlebih dahulu siapa saja Guru yang pendidikannya belum S1, melalui pencarian data individu guru yang belum S1.
• Untuk melaksanakan program peningkatan kompetensi, misal pada tahun 2007 LPMP Provinsi Jambi pernah melaksanakan Diklat Penulisan Karya Tulis Ilmiah bagi Guru berpendidikan minimal S1 dan telah memiliki pangkat/golongan IV/a. Dengan SIM NUPTK secara mudah dapat dilakukan pencarian data individu Guru yang bersangkutan, dan dilakukan pengurutan sesuai peringkat terlama di golongan IV/a. karena dengan diklat ini diharapkan Guru mampu menulis karya ilmiah sebagai persyaratan untuk kenaikan pangkat ke golongan IV/b.
• Untuk melaksanakan program pemberian tunjangan kemaslahatan bagi guru yang akan pensiun, maka sebelumnya ditentukan terlebih dahulu data individu guru yang memiliki usia 1 tahun akan pensiun, hal ini dapat dilakukan dengan pencarian data melalui SIM NUPTK kemudian data tersebut di edit melalui aplikasi lain, seperti excell untuk menyesuaikan format yang ditentukan.
• Untuk melaksanakan program peningkatan kompetensi guru yang dilaksanakan oleh instansi/unit lainnya, LPMP Provinsi Jambi juga telah memberikan layanan data individu guru sesuai dengan kualifikasi, latar belakang pendidikan, dan mengajar bidang studinya. Tinggal bagaimana instansi tersebut memanfaatkan data yang telah diberikan (misal : data Guru matematika, Guru bahasa Inggris, Guru bahasa Indonesia, Guru Penjas, dan BK, dan sebagainya).

Begitu banyak manfaat data SIM NUPTK…. namun yang perlu diperhatikan adalah bagaimana agar data SIM NUPTK yang dikelola LPMP Provinsi Jambi benar-benar valid dan up to date. Untuk itu LPMP Provinsi Jambi dalam hal ini Seksi Program dan Sistem Informasi (PSI) telah melakukan validasi data (untuk tahun 2008 telah dilaksanakan 2 kali) proses ini dilakukan bekerjasama dengan Dinas Pendidikan dan Kantor Depag Kabupaten/Kota mengingat hasil rekomendasi dukungan pelaksanaan pendataan dari Dinas Pendidikan Provinsi Jambi dan Kanwil Depag Provinsi Jambi dengan surat Nomor : Kw.05.4/2/PP.01.1/5140/2008 tanggal, 15 juli 2008.

Pada bulan Juli 2008 validasi data kedua dilakukan oleh Seksi PSI dengan cara mencetak hasil data guru yang telah terentri (print out) selanjutnya dikirimkan ke sekolah melalui Dinas Pendidikan Kabupaten/Kota untuk diverifikasi kebenarannya oleh Guru yang bersangkutan. Bagi guru madrasah karena pada tahun 2008 terjadi proses penetapan status Madrasah Ibtidaiyah maka Madrasah mengisi ulang format NUPTK yang dikoordinir oleh Kantor Depag Kabupaten/Kota.

Hasil pengumpulan data baik oleh Dinas Pendidikan maupun Kantor Depag Kabupaten/Kota akan dikumpulkan oleh petugas LPMP Provinsi Jambi pada bulan Agustus 2008 untuk selanjutnya dilakukan verifikasi dan validasi ulang yang selanjutnya pada akhir tahun 2008 akan menghasilkan data up to date.

Secara total data yang telah terentry oleh LPMP Provinsi Jambi berjumlah 60.752 PTK (keadaan bulan Juli 2008) dari data tersebut 85% telah memperoleh NUPTK, sisanya 15% dalam proses atau data tidak lengkap. untuk itu perlu perbaikan lebih lanjut. LPMP Provinsi Jambi (Seksi PSI) sebagai pelaksana validasi data mengharapkan pada validasi data kedua nantinya akan semakin baik hasilnya sehingga berguna untuk pelaksanaan program tahun 2009.

Atas dukungan semua pihak demi terlaksananya program SIM NUPTK LPMP Provinsi Jambi, kami mengucapkan terima kasih banyak dan menghimbau kepada Pendidik dan Tenaga Kependidikan kiranya masih tetap bersedia dan tidak bosan-bosan untuk memperbaiki datanya, perlu pula disampaikan bahwa sistem pendataan NUPTK tetap mengacu pada perbaikan berkelanjutan artinya untuk tahun berikutnya dilakukan validasi data minimal 2 kali setahun untuk meng-up date perubahan data PTK misal: pangkat/golongan, perubahan status pegawai, keluarga, diklat kompetensi yang pernah diikuti, lulus atau belum lulus sertifikasi, dan sebagainya.
